Failure Analysis Configuration

Failure analysis is a logical and systematic examination of an asset component and/or its documentation, in order to detect and analyse the causes, probabilities and consequences of actual or potential failure.

Effective failure analysis will help prevent common component failure modes from occurring by identifying the following:

  • The underlying cause of the component failure.
  • Preventative tasks required to prevent future failures.
  • Repair types required in the event of the failure.

For example, say an electrical motor is not turning because the bearing has seized as a result of poor lubrication. The failure analysis for this motor could be described as follows:

  • Failure mode: Electrical motor not turning
  • Failure mechanism: Seized
  • Failure cause: Insufficient lubrication
  • Repair type: Replace bearing and lubricate
  • Tasks: Inspect bearing lubrication.

Industry standards for each of the elements of failure analysis are maintained in On Key, so that failure analysis can be performed when an asset component fails and the asset stops operating. The results are recorded on the work order that is raised to repair the asset component.
